refactor: rename files to match pluralized namespaces

The pluralize refactor renamed the namespaces but left the files singular.
Rename to match: stream.ts -> streams.ts (Streams), iterable.ts ->
iterables.ts (Iterables), content.ts -> content-parts.ts (ContentParts,
matching the sibling stream-parts.ts). Internal-only; public entry points
and exports are unchanged.
