Implement `opt_getinlinecache` (most common missing instruction)

This is currently the most common missing instruction for railsbench and RDoc.

The first thing I'd like to ask is: can somebody explain precisely what it is that opt_getinlinecache does? What is the key used to look things up in the IC?
