Location should be specified in side_effect_exprt, having it as an argument of the constructor ensure we don't forget about it.

What does the default source_locationt mean? This feels like pushing the problem one rung down? I suppose it is correct here as this is before we've given every instruction a location so this will get updated in a later sweep?

Not sure, and it could be something that has to be fixed, but at least this PR makes it more obvious that we are doing something unsafe here.

side_effect_exprt normally have a source location associated but this wasn't strictly enforced. This adds a constructor asking for a source location and deprecate the others.
