Most items get automatically added to source control when you create them. Business processes on the other hand you have to remember to click the "Add to Source Control" option on. This is an easily forgotten step.

I'd give it an 80% chance that this is an upstream issue with hooks (specifically the "new document saved for first time" event) - hopefully not though / at least something we can work around
