feat: add MCP tools to Twilio realtime example

Summary

• update the Twilio realtime demo to use the same sample MCP tools as the Next.js realtime demo
• log available MCP tools and auto-approve sample tool calls when bridging to Twilio Media Streams
• document the shared MCP tooling in the example README

Testing

• pnpm -F realtime-twilio build-check
