Improve the abuse reporting page with advanced structured form with interactive coaching

We shall re-work the abuse reporting page by providing a clear, interactive guidance with advanced structured web form.

I envision the abuse reporting page to provide:

• Reason for reporting (select from drop-down)
• Full identity details, including contacts
• Jurisdiction where the Law concerned the abuse apply (US, DE, ETC)
• Legal reference about law being broken
• If the report has been already been sent to some other Tor2web owner
• If the report has been already been sent to the ISP and/or Provider hosting the server
• Action being requested (select from drop-down)
  For each action being requested, the end-user will be given an explaination of what can be done and what cannot be done (like blocking, like tracing, etc, etc)
